Wellbeing Connectors aim to:

  • Improve social and emotional wellbeing
  • Promote independence
  • Reduce social and emotional isolation and loneliness
  • Help people feel part of the community
  • Coordinate appropriate support services to facilitate positive outcomes for complex cases
  • Provide information and advice on suitable community groups and activities

Physiotherapy

The First Contact Physiotherapy Service:

Individuals are able to access assessment by a physiotherapist directly within the GP practice rather than seeing their GP first if they have a concern regarding a musculoskeletal problem and its management.

 The service will provide

An appropriately detailed assessment of the individual and their MSK complaint by an experienced clinician in MSK medicine

 And help to support appropriate decision making and advice on:

  1. Short and long-term self-management recommendations
  2. The need for onwards referral to therapy services (physiotherapy, podiatry etc)
  3. The need for further investigations (such as x rays, MRI scans, blood tests) to support the assessment and decision making process
  4. The need for onwards referral for opinion and intervention from a secondary care consultant service (Orthopaedics, Rheumatology)

 

Aneurin Bevan University Health Board has also developed this website to help the community of ABUHB to look after their bones, joints and muscles.
