Before filling out a registration form, check you are in our catchment area which includes Risca, Pontymister, Cross Keys, Pontywaun, Wattsville, Cwmfelinfach, Ynysddu and Rogerstone (Southwards down to and including St John's Crescent).

Please visit reception from 8am to 6pm and collect a new patient registration form and a new patient questionnaire.
